Consent To Receive SMS Text Messages :
If you sign up to receive text messages from us on any of our Sites, you are consenting to receive text messages related to your relationship with Body and Brain Integrative Care. We value your privacy and want to ensure that you are fully informed about the types of communications you may receive from us.

Terms and Conditions:
Voluntary Consent. Your consent to receive SMS text messages is voluntary. You have the right to refuse, and it will not affect your ability to obtain treatment or benefits.

Message Frequency. You may receive periodic messages based on the information and promotions we believe are relevant to you. Message frequency may vary.

Data Storage. Standard message and data rates may apply to any messages sent from us. Check with your mobile provider for details on your service plan.

Opt-In Option. You may opt-in of receiving SMS text messages by:

Express Consent. Receiving verbal or written consent. For example, text messaging us first.

Express Written Consent. Check the boxes of your communication preferences and sign the consent form found in the initial intake.

Opt-Out Option. You may opt-out of receiving SMS text messages at any time by replying “STOP” to any message you receive.

Privacy. You will protect your personal information in accordance with our Privacy Policy. Your phone number will not be shared with third parties marketing purposes without your explicit consent.
We do not guarantee the successful delivery of text messages by your wireless provider. Messages sent by text may not be delivered if the mobile device is not in range of a transmission site, or if the network is down. Factors beyond the power of wireless carriers may get in the way of message delivery. This may include the terrain, how close you are to buildings, foliage, weather and your equipment. We and your wireless provider will not be liable for losses or damages that come from: a message not delivered, a message delivered late, or a message that goes to the wrong number; or inaccurate or incomplete content in a text message.
We are not liable for your use of reliance on the content of any text message.
If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
Privacy and Security
Please update us right away if your mobile number changes. It is your responsibility to give a correct mobile number and to update any changes. If you don’t give us your new mobile number, we are not responsible for any text message sent to the wrong number.
We recommend that you use a password to open your mobile device.
Since text messages are unencrypted, there is a risk that protected health information (PHI) could be intercepted or viewed by third parties, including others who look at your device. When you choose to get text messages from us, you do so at your own risk. This use and disclosure of PHI in text messages is governed by HIPPA Notice of Privacy Practices.

Body & Brain may use text messaging for issues not related to PHI. Text messages sent to Body & Brain that include PHI may be responded to by using a HIPAA-compliant method of communication.
Check and sign the consent form in the initial intake for your preference to receive communication, promotional information and updates from Body & Brain via SMS text messages to the phone number you have provided.
